The patient underwent Endoscopic Sleeve Gastroplasty (ESG) in January 2022, resulting in weight loss initially. However, a year after the procedure, she experienced weight regain, reaching 230 lb four months post-partum in February 2024. The patient also underwent an Upper GI Series due to postpartum postprandial pain. Management options for weight regain included EGD with suturing and the addition of anti-obesity medication.<br /><br />The case raises important clinical questions regarding the optimal management of weight regain post-ESG, such as choosing between EGD with suturing or medication management. Additionally, the experience of patients who become pregnant after ESG and the use of GLP-1 like semaglutide post-procedure were discussed. Dr. Akagbosu shared insights on potential considerations and guidance for healthcare professionals facing similar cases.<br /><br />This presentation underscores the challenges and considerations in managing weight regain following ESG, especially in patients with comorbidities and unique life circumstances like pregnancy. Further research and experience sharing are needed to optimize the long-term outcomes of patients undergoing ESG for weight management.
