Pdf Summary
The ASGE Annual Advanced Practice Provider (APP) Course 2022 will focus on therapeutic endoscopy and what APPs need to know about it. The course will be presented by Dr. Vivek Kaul and will cover various aspects of therapeutic endoscopy, including its definition, how it differs from diagnostic endoscopy, and the additional training required to become a therapeutic endoscopist. The course aims to improve communication and collaboration between APPs and therapeutic endoscopists.

Therapeutic endoscopy is a subspecialty within the field of gastroenterology that involves the use of endoscopy to perform interventional procedures. It originated in the 1960s with colonoscopic polypectomy and in the 1970s with ERCP and sphincterotomy. To become an advanced interventional endoscopist, one must complete 4 years of medical school, 3 years of internal medicine residency, 3 years of GI/Hepatology fellowship, and 1 to 2 years of Advanced Endoscopy fellowship. However, there are alternative training pathways available, and the content of AE fellowships may vary between programs.

Advanced endoscopic procedures commonly performed include ERCP, EUS, endoscopic resections (EMR, ESD, EFTR), endoscopic ablation, and enteral stenting. These procedures require the expertise of not only the physician but also a well-trained nursing staff, anesthesia, surgery, oncology, radiology, intervention radiology, and the APP team members who play a crucial role in referral triage, pre and post-procedure management, and complication management support.

Advanced endoscopy procedures require specialized equipment and resources, such as specialty endoscopes, endoscopy suites, fluoroscopes, specialty electrosurgical generators, and related devices.

The course also highlights the role of APPs on the advanced endoscopy team, including complex endoscopy referral triage, cognitive clinical assessment, peri-procedure support, interdisciplinary care coordination, and communication with patients, families, and other healthcare teams. Informed consent is emphasized as an essential part of advanced endoscopy, and APPs may be involved in clinical research and scholarly activities within the advanced endoscopy team.

Overall, the course aims to educate APPs about therapeutic endoscopy, enhance their knowledge and skills in this subspecialty, and promote effective collaboration within the advanced endoscopy team to provide high-quality patient care.
