Pdf Summary
This document provides an overview of therapeutic endoscopy and the role of advanced practice providers (APPs) in this field. Therapeutic endoscopy refers to advanced endoscopic procedures that go beyond diagnostic purposes. Terms such as interventional endoscopy, advanced endoscopy, and therapeutic endoscopy are often used interchangeably. The training to become an advanced interventional endoscopist involves 4 years of medical school, 3 years of internal medicine residency, 3 years of GI/hepatology fellowship, and 1 to 2 years of advanced endoscopy fellowship. Different programs may have variations in the content of their advanced endoscopy fellowships.

Advanced endoscopic procedures include deep enteroscopy, endoscopic ablation procedures, ERCP, EUS, EMR, ESD, EFTR, POEM, bariatric endoscopy, enteral stenting, and endoscopic "endosurgical" interventions. These procedures require a well-trained and experienced medical team, including physicians, nursing staff, techs, anesthesia, surgery, oncology, radiology, and interventional radiology.

APPs have an important role in the advanced endoscopy team. Their responsibilities include complex endoscopy referral triage, cognitive clinical assessment, shared and independent clinic visits, peri-procedure support, follow-up communication and results management, interdisciplinary care coordination, and complication management. APPs should have a cognitive skillset that includes knowledge of clinical indications and procedures, guideline-based practice paradigms, contraindications, management of anticoagulants, and the implications of altered GI anatomy. They also play a role in informed consent, scholarly activity, and research in advanced endoscopy.

The document emphasizes the importance of a team-based approach and collaboration between APPs and physicians for the safe and successful delivery of advanced endoscopy services. APPs should have ongoing mentorship and professional development opportunities. Proper training, specialized facilities, and additional equipment are required for advanced endoscopy practice.

In summary, therapeutic endoscopy involves advanced procedures beyond diagnostics. APPs have a crucial role in the advanced endoscopy team and should have the necessary knowledge, skills, and support to provide safe and effective patient care.
