ASGE Guideline on Screening and Surveillance of Barrett’s Esophagus

Pdf Summary
The ASGE guideline on Barrett's esophagus recommends surveillance endoscopy for non-dysplastic BE but evidence is of low quality. Screening for BE is inconclusive, but if done, it should target high-risk individuals. Chromoendoscopy and Seattle protocol biopsy sampling are recommended for BE surveillance, while routine use of confocal laser endomicroscopy is not advised. Endoscopic ultrasound is not recommended for differentiating mucosal from submucosal disease. Virtual chromoendoscopy and wide-area transepithelial sampling need further research. Dysplasia detection can be improved with confocal laser endomicroscopy and volumetric laser endomicroscopy, although availability is limited. The study shows higher dysplasia detection rates with VLE laser in post-treatment surveillance but more research is needed on its cost-effectiveness and patient preferences.
