Ahora le pasaré la presentación al Dr. Leggett. Buenas noches. Mi nombre es Catman Leggett junto con Hernando González. Somos co-directores de esta serie de webinars en español del grupo de interés de Latinoamérica de la Sociedad Americana de Endoscopía Gastrointestinal. Les doy la bienvenida a este webinar sobre la inteligencia artificial y su aplicación en el campo de la gastroenterología. Como gastroenterólogo, me interesa mejorar nuestra habilidad de diagnosticar enfermedades a través de la endoscopía. Mi práctica se enfoca en el esófago de Barrett y la aplicación de imaginología avanzada en su diagnóstico. Así es como inicialmente tuve un interés por el campo de la inteligencia artificial, un campo que está creciendo exponencialmente en nuestra disciplina. Comienzo comentándoles que no tengo ningún conflicto de interés relacionado a este webinar. El objetivo principal de hoy es discutir lo que es la inteligencia artificial, también conocida como IA, y resaltar algunos ejemplos de cómo se aplica en el campo de la endoscopía gastrointestinal. También vamos a discutir cuáles son los beneficios de la inteligencia artificial y las limitaciones que tenemos que tomar en cuenta a medida que estos algoritmos se implementan en nuestra práctica. Comencemos describiendo qué es la inteligencia artificial. La inteligencia artificial es omnipresente en nuestra vida cotidiana. Hoy en día hay algoritmos que predicen qué película queremos ver, qué productos queremos comprar y qué música queremos escuchar. Una de las aplicaciones más asombrosas de la inteligencia artificial es el de vehículos automatizados. En el campo de la medicina, la aplicación de la inteligencia artificial es más reciente. Esto se debe en parte a que la información de nuestros pacientes es considerada confidencial y debe de ser protegida. La data médica también suele ser más compleja y requiere de expertos en cada campo para su debida interpretación. Lo que no cabe duda es la cantidad de enorme información que tenemos hoy en día sobre nuestros pacientes, que en algunos casos puede ser algo abrumador. Por lo tanto, la inteligencia artificial promete facilitar la interpretación de la data médica. Lo logra al encontrar patrones que resumen la data de una manera eficiente. Muchas veces estos patrones son solo reconocidos por la computadora y pueda que no sean aparentes para el ser humano. El campo de la inteligencia artificial no es nuevo y se inicia en los años 50's, donde los primeros algoritmos que imitan funciones cognitivas fueron creados. Lo que ha permitido que la inteligencia artificial se implemente en nuestra vida cotidiana es el desarrollo de computadoras con un poder de procesamiento mucho más avanzado. Hoy en día, nuestro teléfono celular tiene una capacidad de procesamiento mucho mayor a la de las computadoras en los años 50's. En los 80's se desarrollan algoritmos más avanzados que buscan reconocer patrones en la data para completar tareas de una manera automatizada. Sin embargo, en la última década es cuando se desarrolla el tipo de algoritmos conocidos como redes neuronales que nos permiten conseguir un aprendizaje realmente automatizado. Hoy en día, el término inteligencia artificial se usa en referencia a estos algoritmos. Un término más específico es el aprendizaje profundo o deep learning como se dice en inglés. En el campo de la inteligencia artificial hay dos ramas que se han desarrollado en paralelo usando este tipo de algoritmos. Una de estas es la visión por computadora en la que un algoritmo es capaz de reconocer patrones dentro de una imagen. Ese es el tipo de algoritmo con mayor aplicación en la endoscopía. La segunda rama es la de procesamiento de lenguaje natural o PLN. En esta rama la inteligencia artificial es usada para encontrar contenido y significado de textos. A continuación discutiremos estas dos ramas de la inteligencia artificial en más detalle. Comencemos por la rama de visión por computadora. Hoy en día existen algoritmos capaces de reconocer objetos como por ejemplo un tomate. ¿Cómo funcionan estos algoritmos? El algoritmo es construido usando una base de datos de imágenes. La más conocida se llama ImageNet y consta de millones de imágenes clasificadas de objetos, animales, plantas, etc. Esta base de datos se usa para entrenar un algoritmo a encontrar diferencias entre cada categoría. El algoritmo es una red neuronal convolucional compuesto de varios nódulos y capas. En cada capa patrones específicos de la imagen son comparados y asignados un valor proporcional. Las primeras capas puedan que definan líneas, curvas o sombras y se van especializando hasta llegar a capas más profundas donde el objeto es reconocido y clasificado. Si queremos entrenar un algoritmo para reconocer un tomate por ejemplo, entonces también debemos mostrarle imágenes más específicas de frutas y verduras que se asemejen. De esta manera el algoritmo aprende la diferencia entre un tomate y una manzana por ejemplo. De ahí la performancia del algoritmo es comprobada al mostrarle una imagen de un tomate que no ha visto antes. Esta se denomina la imagen de entrada. El algoritmo procesa los píxeles de la imagen, encuentra un patrón y lo compara con los patrones que pudo descubrir en las imágenes de entrenamiento. La capa de salida clasifica la imagen y proporciona un porcentaje de precisión. En este caso el algoritmo está 95% seguro de que la imagen mostrada es un tomate. La aplicación de redes neuronales convolucionales a imágenes endoscópicas no es muy diferente. Se necesita una base de datos de imágenes de endoscopía que son clasificadas y capaces de encontrar patrones para un sinnúmero de enfermedades. A continuación revisaremos algunos de los algoritmos que han sido diseñados. Es importante tomar en cuenta este esquema cuando uno habla de algoritmos de inteligencia artificial. Todo algoritmo comienza con una base de datos de imágenes endoscópicas. El tamaño de esta base de datos es importante. Cuanto más imágenes, mejor. Estas imágenes son anotadas usualmente por expertos en el campo. La valorización puede ser ticótoma, en otras palabras, si la condición está presente o ausente. También puede incluir una delineación de una lesión. En este proceso es importante tomar en cuenta cuál es el estándar dorado, ya que esto dictamina la performancia del algoritmo. En otras palabras, estamos buscando una performancia comparable a expertos en el campo, en casos de un diagnóstico visual o a histopatología en casos de un diagnóstico patológico. La base de imágenes se divide en un set de entrenamiento, que es usualmente el 80% de las imágenes, y un set de validación, que es usualmente el 20% de las imágenes. Es importante tomar en cuenta que una validación externa es fundamental. En otras palabras, la performancia del algoritmo debe ser comprobada usando una base de datos externa a la usada para su entrenamiento y validación, ya que estos algoritmos son tan sensibles que pueda que capten sutiles diferencias de un set de imágenes que no existe en otra. La performancia del algoritmo es medida en la precisión de diagnosticar una enfermedad, así como la sensibilidad y especificidad, lo que también se reporta como la curva de característica operativa del receptor o curva ROC. A continuación, resumiremos algunas de las aplicaciones de la inteligencia artificial a la endoscopía gastrointestinal. Cuando practicamos endoscopía, a un nivel casi inconsciente, estamos realizando dos pasos importantes. Primero es la detección o identificación de una lesión, por ejemplo un pólipo. De ahí sigue el diagnóstico o caracterización de la lesión. En el caso de un pólipo, si es hiperplástico o adenoma. El uso de algoritmos automatizados genera los siguientes términos para estos pasos. Los algoritmos CAD-E son algoritmos que sirven para la detección de una lesión. Los algoritmos CAD-X son algoritmos que sirven para la caracterización de la misma. Comencemos discutiendo la aplicación de este tipo de algoritmos en el esófago de Barrett. Aquí les proporciono una imagen de un paciente con esófago de Barrett. Un algoritmo de tipo CAD-E es utilizado para resaltar o demarcar la lesión. Y puede que también proporcione el lugar donde uno debe de tomar una biopsia. Este tipo de algoritmo fue diseñado por un grupo de investigadores en Amsterdam usando una base de medio millón de imágenes endoscópicas y refinado con imágenes de pacientes con esófago de Barrett. La anotación de estas imágenes fueron hechas por expertos que clasificaron las imágenes como imágenes con o sin displasia y delinearon las imágenes correspondientes en pacientes con esófago de Barrett y displasia. La precisión del algoritmo fue de 88% comparada a la precisión de expertos que fue de 73%. Lo impresionante es que el algoritmo logró delinear la lesión en todos los casos de esófago de Barrett con displasia y coincidió con exactitud a la delineación de los expertos. El mismo grupo de investigadores desarrollaron un algoritmo CAD-X para caracterizar una lesión con el uso de imágenes de banda estrecha. En este caso, el algoritmo fue entrenado con imágenes de banda estrecha obtenidas en pacientes con esófago de Barrett con y sin displasia. La notación de las imágenes fue determinada por la histopatología de biopsias correspondientes a la imagen. La precisión de este algoritmo para determinar si una lesión contiene displasia fue de 84%. El grupo de investigadores también validó el uso del algoritmo usando videos de endoscopía en pacientes con esófago de Barrett con una performancia similar. Existen otros estudios similares tanto en esófago de Barrett como en carcinoma escamoso en etapa temprana. Este meta-análisis reciente resume la performancia de estos algoritmos. El meta-análisis contiene cuatro estudios en esófago de Barrett, los cuales obtuvieron una sensibilidad de 89% y especificidad del 89% para el diagnóstico de lesiones neoplásicas. También contiene cuatro estudios de carcinoma escamoso temprano, los cuales obtuvieron una sensibilidad de 96% y especificidad del 94%. Los autores de este meta-análisis concluyen que la aplicación de la inteligencia artificial para la detección de cáncer de esófago y lesiones premalignas tiene una alta precisión diagnóstica, pero requiere de mayor validación externa para determinar su uso clínico. El siguiente algoritmo es un ejemplo de la aplicación de la inteligencia artificial en el diagnóstico y caracterización de varices gastroesofágicas. El algoritmo de este estudio fue entrenado usando más de 8,000 imágenes de pacientes con varices y comparadas a más de 6,000 imágenes de pacientes sin varices. El algoritmo fue validado usando un set de más de 4,000 imágenes. Las imágenes fueron anotadas por expertos en el campo por presencia o ausencia de varices. También se identificaron si factores de riesgo como tamaño, erosión y signo rojo estaban presentes en aquellas imágenes con varices. De esta manera se generó un algoritmo CAT-E para diagnosticar la presencia de varices gastroesofágicas y un algoritmo CAT-X para clasificarlas como varices de alto o bajo riesgo de sangrado. El algoritmo también proporciona una opinión sobre si el tratamiento de varices es necesario. La precisión del algoritmo fue superior a la de los expertos con una precisión de 97% para el diagnóstico de varices y 84% para la caracterización de factores de riesgo. El algoritmo obtuvo una precisión de 91% en sugerir si el tratamiento de varices era necesario. La inteligencia artificial también ha sido aplicada a la endoscopía capsular. En este estudio millones de imágenes fueron usadas para entrenar un algoritmo a detectar un sinnúmero de lesiones del intestino delgado. El algoritmo obtuvo una performancia impresionante con una sensibilidad de 99.8% comparada a 74.6% en expertos. El tiempo de lectura del algoritmo fue de 5.9 minutos comparado a 96.6 minutos por expertos. Este algoritmo fue diseñado para ejecutar con una alta sensibilidad ya que es un algoritmo decribado. Sólo el 3% de las imágenes detectadas por el algoritmo como anormales fueron normales. Un metanálisis reciente resume la performancia de 10 algoritmos para clasificar lesiones del tracto digestivo delgado usando endoscopía capsular. En resumen, 5 de estos algoritmos fueron diseñados para detectar úlceras con una sensibilidad de 95% y especificidad de 94% y 5 algoritmos para detectar sangrado con una sensibilidad de 98% y una especificidad del 99%. La aplicación de la inteligencia artificial a la endoscopía capsular obtiene una excelente precisión diagnóstica en la detección de estas lesiones, pero hay que tomar en cuenta que los estudios realizados hasta el día de hoy son todos retrospectivos y su capacidad de diagnóstico clínico aún debe de ser comprobada. Lo que sí no cabe duda es que estos algoritmos pueden revisar un estudio de cápsula con mucho más rapidez que un ser humano. La aplicación de la inteligencia artificial a la endoscopía gastrointestinal con mayor interés es el de diagnóstico de pólipos colónicos. Los algoritmos CAD-E nos proporcionan un diagnóstico automatizado y prometen mejorar nuestra tasa de detección de adenomas. Los algoritmos CAD-X nos proporcionan un diagnóstico óptico para diferenciar adenomas de pólipos hiperplásticos y cerrados. Esto es importante ya que la Asociación American Association of Gastrointestinal Endoscopy recommends that if an optical diagnosis of a tiny polyp, that is, of 5 millimeters in size or less, is obtained with a certain degree of certainty, that these polyps can be diagnosed without being resected in the case of hyperplastic polyps, or resected and discarded in the case of being adenomas. This strategy for the management of adenomas has the economic benefit of reducing the cost of a colonoscopy. Here is an example of how one of these algorithms works. In the image shown, a tiny polyp is found. CAT-E type algorithms are used to detect colonic polyps, highlighting their location using a square superimposed on the image. There are six random control studies for the detection of colonic polyps. This is an impressive number of random control studies, and without a doubt the largest number of studies of any artificial intelligence application in medicine. The results of these studies were summarized in two recently published meta-analyses. Here are the data from one of these meta-analyses, since both had similar results. In the six studies, a total of 2,146 patients obtained colonoscopy assisted by an algorithm, and 2,165 without the assistance of an algorithm. A total of 2,045 polyps were detected using the algorithm, and 1,130 without the algorithm. Two of these studies also used quality control algorithms, and we will talk more about this type of algorithms below. The meta-analysis determined that the rate of detection of adenomas in chronoscopies assisted by artificial intelligence was 29.6% compared to 19.3% in those without assistance. In the same way, the rate of detection of polyps was 45.5% in the group that obtained assisted chronoscopy compared to 30.6% without artificial intelligence. The use of artificial intelligence algorithms only increased the time of the procedure by 30 seconds. However, it should be noted that the increase in the detection of adenomas by algorithms was in small adenomas and had no impact on larger adenomas or advanced adenomas. There are also polyp characterization algorithms using narrow band images. In this study, a narrow band classification was used to differentiate between hyperplastic polyps and adenomas. The algorithm was trained and validated using a database of videos. The annotation was described by experts as type 1 versus type 2 using this classification. In other words, if the polyp is hyperplastic or adenoma. This algorithm obtained a precision of 94% per injury in the classification of adenomas. In a recent podcast in Spanish, Hernando and I talked to Gabriel Maicas, a systems engineer who designed a CAD-X type algorithm to differentiate closed injuries in a colonoscopy. If you are interested, I recommend that you look for it on the GI page under podcast in Spanish. As I mentioned, there are also algorithms that measure the quality of the colonoscopy since it also has a direct impact on the detection of adenomas. The algorithm called EndoAngel uses annotated images of the quality of the intestinal preparation using the Boston Bowel Preparation Score. The algorithm's precision was 93% compared to 75% in experts. The algorithm's performance decreased with the use of videos, but remained above 80%. Another algorithm is the Quality Control Automatic System. This algorithm measures the stability of the colonoscopy during withdrawal, withdrawal time, image quality, and intestinal preparation quality. The algorithm increased the detection of adenomas, but with an increase in withdrawal time. The last colonoscopy application that I would like to share with you is the evaluation of the degree of inflammation in ulcerative colitis. This group of researchers used more than 40,000 images of patients with ulcerative colitis to train an algorithm to detect the degree of inflammation and whether patients were undergoing endoscopic and histological remission. The algorithm's precision to evaluate the degree of inflammation was comparable to experts and histopathology without the need to obtain a biopsy. Next, we will discuss the application of natural language processing to gastrointestinal endoscopy. This is the other branch of artificial intelligence that is being used in the field of medicine. Natural language processing refers to the ability of an algorithm to understand the content of a document. The challenge is that human language is not structured in such a way that a computer can easily extract content. For example, polysemic words, those that are written the same but have different meanings, can confuse an algorithm. In the example, the bank is blue. Without the appropriate context, we do not know if it refers to a bank to settle or a financial bank. In an appropriate context, a human being is capable of knowing the difference, but this is not so easy for a computer. The challenge in medicine is that we constantly describe the same thing in different ways. For example, the terms rectal bleeding and hematopoiesis can be used in a similar way. How has PLN been used in gastrointestinal endoscopy? I will share a couple of examples. In this study, PLN was used to extract data from more than 300,000 colonoscopies of approximately 250,000 patients. In this group of 71 patients, they were diagnosed with closed polyposis syndrome using the criteria of the World Health Organization. Only 25 patients had an established polyposis diagnosis, of which PLN was able to detect this syndrome 2 years before the clinical diagnosis in 5 patients. This study emphasizes how artificial intelligence can be used to extract specific patterns from such a large database that cannot be manually reviewed. PLN has also been used to measure the quality of documentation in colonoscopy. In this study, a PLN algorithm was used to extract data from 24,000 reports from colonoscopy and pathology in 9 hospitals. The quality of colonoscopy in these institutions was reported automatically and it was determined that manual documentation was appropriate only in 62.7% of the cases. The adenomas detection rate was also automatically calculated. This type of algorithm facilitates the quality control of colonoscopy as it avoids the redundancy of manual review of these documents. In the last part of this webinar, I would like to talk a little about the benefits and limitations of artificial intelligence. The application of artificial intelligence to gastrointestinal endoscopy is here to stay. This table shows the number of polyp detection algorithms that are being approved for clinical use. The vast majority of these algorithms have already been approved in Europe and Asia. And here in the United States, the FDA announced last week the approval of the first artificial intelligence system for the diagnosis of colonic polyps by Medtronic called GI Genius. So we have to start thinking about how we are going to incorporate these systems into our practices. Number one, if it is safe to do so, and what is the cost-benefit ratio of these products? At the moment, many of these questions remain unanswered. As artificial intelligence becomes more and more common in our field, it is important to keep in mind that these algorithms are not perfect and may lead to medical errors. There are several limitations to the artificial intelligence that we must keep in mind. First of all, the ability of an algorithm depends on the quality of the data it was trained with. In the vast majority of algorithms, the data is written down by experts in a field. However, we may be introducing errors and biased data without necessarily knowing it. In this way, the algorithm may be learning how to do something as an expert, but the expert may be wrong. This brings me to the second point, which is that the vast majority of these algorithms work in a way that it is not known why the algorithm reaches a particular diagnosis, which is sometimes referred to as the black box of artificial intelligence. Today, there is a lot of interest in the field of artificial intelligence to develop algorithms that provide the ability to explain how to reach a diagnosis. There are also many questions about how algorithms are going to be regulated, since many of these constantly use data to improve their performance. At the same time, it is not known the legal ramifications that these algorithms may have if a medical decision was made based on them. Finally, it is important to bear in mind that the data that these algorithms handle is confidential and that a violation of a patient's personal data can have very serious consequences. Despite these limitations, artificial intelligence has a lot to offer in our field. Not only does it have the ability to improve our clinical diagnosis, but it also provides us with a panoramic view of the health of our patients. It allows us to predict results and measure risks, and thus improve the care of our patients. In the case of endoscopy, it gives us an immediate second opinion that allows us to make decisions on the spot. Finally, the amount of data that a single patient produces today is overwhelming. Artificial intelligence will allow us to be more efficient and reduce the fatigue associated with the manual review of this data. And with this I conclude this webinar on artificial intelligence in endoscopy. I hope you liked it and I thank you for your attention. Our first question is, will the artificial intelligence application change the detection interval for colonoscopy? Thank you, Reddy, for the invitation and for being here with all of you, talking about this relatively new field, but the truth is that it has advanced a lot in recent years. Your question about whether artificial intelligence will be able to change the interval of colonoscopy is a good question, and the truth is that the answer is not known. One could say that if one has an algorithm that ensures that all polyps are being detected in a colonoscopy, that one could postpone the necessary time for the next colonoscopy. But clinical studies are really needed on how these types of algorithms will behave in a context of daily use. All the studies that have been carried out to date are high-quality studies, but they are studies to measure the effectiveness of algorithms and not necessarily their clinical use. Thank you, excellent answer. Any comments on the reimbursement of services provided through artificial intelligence? Another excellent question, thank you, Reddy. There are a lot of questions about whether there will be some kind of reimbursement for the use of this technology. Obviously, there is a cost associated with the use of technology. For example, the system that has just come out of Medronix, the GI Genius, will be based on a subscription. So, one will have to pay, I'm not sure if it's monthly or annually, a subscription for the use of the algorithm, which increases the cost of the procedure. Until now, it is not known if there will be a kind of remuneration for the use of this technology. It is an algorithm that the way it is executed is that it should learn over time, that is, it should get better over time. But the way the FDA regulates many of these algorithms is that the algorithm has to be analyzed, it has to be analyzed in a certain time. So, one cannot generate changes to the algorithm without being approved again. So, there is a system of regulation behind these algorithms that is also associated with this cost. Well, another question. Yes, thank you. I see a question about the estimated cost that a colonoscopy would have. We did this calculation with the GI Genius here in our practice, and we calculated that the cost of a colonoscopy would increase by about $30, the use of the system, which should be considered in practices of different sizes. That is basically a calculation that we have made. I am not saying that it is accurate for all practices. Thank you, Dr. Leggett. Final comments for Artificial Intelligence. Well, I have a final comment. Thank you, Reddy, for the invitation to be here with you. Thank you to all the audience for your time. The truth is that it is a field that is growing, it is advancing very quickly. The impact it will have clinically will still be determined, and there are many investigations that must be done regarding it. The applications that are emerging, as I have shown in this webinar, are for different types of diseases and diagnostic utility. What I hope will happen as this field advances, is that many of these algorithms begin to conglomerate, and there is basically an algorithm that allows us to do all this together, instead of having separate algorithms, that all this is done together. I believe that this will be possible. I know that, for example, Olympus is thinking of doing this with the new version that they are releasing, but that has not yet reached clinical use here in the United States.
