Video Summary
In this video, a case of a 48-year-old woman with FAP who had a subtotal colectomy and ampulectomy over 10 years ago is presented. The patient is admitted with necrotizing pancreatitis and undergoes CT scans which show a large peripancreatic collection. The patient is sent for interventional radiology drainage and also develops a DVT and is started on anticoagulation. The patient continues to have fevers and inadequate drainage, leading to referral for endoscopic drainage. The video discusses the indication for drainage, preferred endoscopic techniques, and the use of EUS-guided drainage with luminoposing metal stents or double pigtail stents. Additional procedures, such as necrosectomy and treatment of complications like bleeding pseudoaneurysms, are also discussed. The panel provides their viewpoints on scheduling necrosectomy procedures and the importance of imaging and ongoing management.
