In this video, Ara Sahakian presents a case of early gastric cancer in a 74-year-old Korean female. The patient had an upper endoscopy that revealed a subtle erosion at the angularis of the stomach, which was biopsied and showed adenocarcinoma. Further staging was performed through CT scans and endoscopic ultrasound, which showed no evidence of mucosal invasion or lymphadenopathy. The patient was presented with the options of a local resection or total gastrectomy. ESD resection was performed using traction to overcome the challenges of the location. Pathological diagnosis confirmed well-differentiated intramucosal adenocarcinoma, and a curative resection was achieved. Post-treatment surveillance showed no recurrence.
