Because colonic EFTR for lesions >20 mm carries higher adverse-event risk and higher local residual rates than ESD, the team chose STER. EUS showed a hypoechoic tumor from the muscularis propria with partial extraluminal growth. A submucosal tunnel was created, but the tumor could not be separated from muscle, requiring full-thickness incision. The specimen was removed, the mucosa preserved, and the defect closed with clips. Pathology: leiomyoma. Recovery was uneventful.
