A 65-year-old woman with Bouveret syndrome, a rare cause of gastric outlet obstruction from an impacted gallstone, presented with nausea, vomiting, and epigastric pain. CT showed a cholecystoduodenal fistula and a large stone lodged in the duodenum. After standard endoscopic methods failed, including snares, balloons, a helix device, and lithotripsy, clinicians used a flexible cryoprobe to freeze-adhere and remove the stone endoscopically. The stone was then fragmented in the stomach and removed. She recovered without complications, highlighting cryoprobe-assisted extraction as a promising minimally invasive rescue technique for high-risk patients.
