Video Summary
This case series describes cholangioperitoneoscopy for complex post-cholecystectomy bile duct injuries. In three patients with persistent bile leaks after surgery, standard ERCP failed because wires could not access the intrahepatic ducts and often entered peritoneal collections. Using cholangioscopy through leaks, fistulas, or collections, the team identified ligated or clipped ducts, regained biliary continuity, and placed stents and drains. In one case, a fistula was also used to treat an unsuspected hydatid cyst. The approach controlled leaks, resolved symptoms, and in some cases converted urgent surgery into elective reconstruction, or potentially avoided surgery altogether.
