A 71-year-old man with prior esophageal cancer underwent routine upper endoscopy with systematic examination of the oral cavity, oropharynx, and hypopharynx using white light and narrow-band imaging. A subtle 4 cm hypopharyngeal lesion, barely visible on white light, was better detected with NBI and biopsied as high-grade dysplasia. The patient underwent endoscopic submucosal dissection with marking, injection, and clip-line traction, achieving complete en bloc resection. Pathology showed in-situ squamous cell carcinoma with clear margins and no lymphovascular invasion. Recovery was uneventful, and follow-up showed no symptoms, supporting careful screening and ESD as an organ-preserving treatment.
