Video Summary
This video presents two rare cases of freehand endoscopic full-thickness resection (EFTR) for distal duodenal subepithelial lesions, both GISTs. The procedure was challenging because of the duodenum’s location, vascularity, and difficult defect closure. In one case, preoperative imatinib reduced tumor size before resection. Both lesions were removed en bloc with careful dissection, then the full-thickness defects were closed using endoscopic suturing plus clips. Prophylactic intraluminal endoscopic vacuum therapy and nasojejunal feeding were used to reduce leak risk and support healing. Follow-up endoscopy confirmed good recovery in both patients.
