A 50-year-old woman with complex prior gastric surgeries had refractory GERD, dumping syndrome, and severe postprandial hypoglycemia. During endoscopy, the team performed band-assisted EMR at the gastroesophageal junction, then created a functional neopylorus using endoscopic full-thickness suturing with multiple reinforcing stitches. They also performed plication to narrow the gastroesophageal junction. The procedure aimed to improve gastric emptying and reduce reflux and hypoglycemia. At six-month follow-up, her hypoglycemia nearly resolved and GERD was controlled on once-daily PPI therapy, suggesting endoscopic neopylorus creation with resection and plication can be safe and effective.
