Video Summary
A 75-year-old man with complete dysphagia from a 6-cm fibrotic obstruction of the neopharynx and esophagus after laryngectomy and chemoradiation underwent endoscopic recanalization. Using a gastrostomy-assisted, rendezvous-style approach with gastropexy, endoscopes were passed from both the mouth and stomach to identify the obstruction. After saline and indigo carmine injection, a fibrotic septum was incised with an endoscopic knife, creating a new lumen without fluoroscopy. The patient was observed, advanced gradually, and at 6 months remained improved and able to swallow solids.
