A 72-year-old man with dysphagia had a 22 cm pedunculated esophageal mass, later found to be a well-differentiated liposarcoma. Because of its size and bleeding risk, endoscopic removal was challenging. After failed endoloop placement, clinicians used an external suture cinch to compress the stalk, then performed endoscopic dissection with a therapeutic scope pre-fitted with an over-the-scope clip. When bleeding occurred, the clip controlled it successfully. The large lesion was removed in two sessions with piecemeal resection and overtube assistance. He recovered well, with complete symptom resolution and no residual tumor on follow-up biopsies.
