A 50-year-old woman with a frozen abdomen after extensive bowel surgery was no longer a surgical candidate and had severe obstruction with poor quality of life. The team first performed an ultrasound-guided endoscopic gastroenterostomy using lumen-apposing metal stents to decompress the excluded stomach. Later, they restored bowel continuity with a magnet-assisted endoscopic duodenocolostomy. Using gastroscope, colonoscope, fluoroscopy, guidewires, and magnets, they created a 15-mm anastomosis. The patient resumed oral intake, passed stool normally, and improved markedly. This case highlights the potential of advanced therapeutic endoscopy to reconstruct gastrointestinal continuity in complex, high-risk patients.
