During an EDGE procedure in a 62-year-old woman with prior Roux-en-Y gastric bypass and a common bile duct stone, a rare type 3 lumen-apposing metal stent misdeployment occurred: the distal flange entered the excluded stomach, but the proximal flange deployed into the peritoneal cavity. Using a NOTES-based salvage technique with an ultrathin endoscope, wires were re-established, spilled gastric contents were suctioned, and a second LAMS was placed successfully to create a gastroenteric conduit. The patient recovered well, later underwent ERCP with stone removal, and had no leak or major complications.
