Researchers demonstrated that EUS-guided creation of a portosystemic shunt using a small-caliber vascular stent is technically feasible and acutely safe in a live porcine model. The procedure used endoscopic, ultrasound, and fluoroscopic guidance to access the hepatic and portal veins, place a stent, and confirm shunt patency with Doppler and venography. Bleeding was effectively controlled with hemostatic slurry, and necropsy showed no major complications. This approach may offer a minimally invasive option for rapid portal decompression when traditional TIPS is not feasible.
