A 78-year-old woman with prior chemo-radiation for proximal esophageal squamous cell carcinoma developed complete esophageal obstruction and severe dysphagia. Endoscopic ultrasound (EUS)-guided recanalization was performed using a fine-needle aspiration needle to puncture from the proximal to distal lumen, followed by guidewire placement, balloon dilation, and temporary nasojejunal tube placement to maintain patency. Serial dilations improved the tract, but frequent procedures led to placement of a fully covered self-expanding metal biliary stent with anti-migration features. She tolerated the stent well and had no swallowing difficulties for two months.
