Video Summary
This case report describes a 32-year-old woman with recurrent pancreatitis due to pancreatojejunal anastomotic stenosis after pylorus-preserving pancreatogenectomy. Standard double-balloon enteroscopy failed because of altered anatomy. The team used a dual-modality EUS approach: first creating a gastroenterostomy with a lumen-apposing metal stent, then using that access to perform EUS-guided pancreatic rendezvous, dilate the anastomosis, and place a pancreatic stent. After repeat dilation and stenting, the anastomosis remained patent, symptoms resolved, and she stayed asymptomatic for two years. This technique may help when conventional endoscopic access is not feasible.
