This first-in-human case series describes a novel two-stage endoscopic method to permanently reverse Roux-en-Y gastric bypass by creating a durable gastro-gastric anastomosis. In six high-risk patients with malnutrition, two lumen-opposing metal stents were placed to form adjacent fistulas, then removed months later and the intervening septum was dissected to create a 2.5–4 cm connection. All procedures were technically and clinically successful, with improved BMI and albumin, durable patency at follow-up, and no major adverse events. The technique may offer a minimally invasive alternative to high-risk surgical reversal.
