Persistent dysphagia after Nissen fundoplication can severely affect quality of life and is often treated with surgical wrap revision or balloon dilation. In this case, a 44-year-old woman developed severe dysphagia and weight loss after surgery. Tests showed a tight fundoplication wrap causing gastroesophageal junction outflow obstruction. Because she declined surgery, doctors performed third-space endoscopic fundoplication release: a submucosal tunnel was created, a myotomy exposed the wrap, and the constricting muscle was divided. The patient improved immediately, resumed oral intake the next day, and remained symptom-free at 3 months with normal follow-up studies.
