This video presents a revisional combined transoral incisionless fundoplication (C-TIF) for refractory GERD after failed primary TIF. A 43-year-old man developed recurrent reflux due to a disrupted wrap and 3 cm sliding hiatal hernia. Laparoscopic hiatal dissection and posterior cruroplasty restored intraabdominal esophageal length, followed by redo TIF with 20 full-thickness fasteners to create a 270-degree, 3 cm fundoplication. Postoperatively, symptoms resolved quickly, with only mild transient dysphagia. At 3 months, imaging showed no recurrent hernia, and he remained symptom-free off acid-suppressive therapy. Revisional C-TIF appears safe, feasible, and effective.
