A man with Crohn’s disease and a frozen abdomen had an impacted gallbladder neck stone that could not be treated surgically. Over eight months, doctors created a cholecystoduodenostomy with a lumen-apposing metal stent, then performed repeated cholecystoscopies and electrohydraulic lithotripsy to reduce stone burden. When the neck stone remained inaccessible, they used an innovative EUS-guided transduodenal 19-gauge needle to deliver lithotripsy directly into the gallbladder. Despite a brief post-procedure cholecystitis, staged repeat treatments ultimately cleared the gallbladder, restored drainage, and left the patient stone-free and symptom-free at one year.
