A 60-year-old woman with worsening dysphagia had a growing 6 cm benign leiomyoma at the gastroesophageal junction. Because the lesion was too large for standard en bloc submucosal tunneled resection, the team performed a piecemeal endoscopic submucosal tunneled resection. They created a submucosal tunnel, dissected the mass, then used snares and a hybrid knife to remove it in pieces while preserving the overlying mucosa. The full-thickness defect was closed with five clips. Pathology confirmed leiomyoma, postoperative imaging showed no leak, and her dysphagia resolved completely.
