A 42-year-old woman developed a severe staple-line leak with a large necrotic cavity after sleeve gastrectomy, causing sepsis and prolonged illness. Despite multiple surgeries and 14 sessions of endoscopic vacuum therapy, the cavity did not heal. Doctors then performed a full-thickness gastric myotomy along the staple line to create open drainage from the cavity into the stomach. The patient improved rapidly, resumed feeding, and was discharged six days later. Follow-up endoscopy five weeks afterward showed complete healing. This case suggests full-thickness gastric myotomy may be a promising option for refractory post-sleeve gastrectomy leaks.
