A 63-year-old woman with recurrent multifocal esophageal squamous dysplasia underwent surveillance and targeted treatment. High-definition white light, narrowband imaging, and near-focus magnification showed no obvious lesions, but Lugol’s chromoendoscopy revealed subtle unstained areas suspicious for dysplasia. These regions were marked with argon plasma coagulation and treated with focal radiofrequency ablation using a Bach 60 catheter. The ablated mucosa whitened uniformly, and biopsies were taken from multiple levels. She tolerated the procedure well without complications and was scheduled for repeat endoscopy in 3–6 months. The case highlights Lugol’s staining as essential for detecting and guiding treatment of subtle squamous dysplasia.
