A 68-year-old woman with jaundice and abdominal pain was found to have biliary dilation, a proximal CBD stricture, stones, and migrated surgical clips after prior ERCP and cholecystectomy. Endoscopic treatment included balloon dilation, cholangioscopy, stone extraction, electrohydraulic lithotripsy, and thulium fiber laser excision to free and remove a retained clip. Three biliary stents were placed. The procedure was successful without complications, liver tests improved, and the patient was discharged on day 3 with planned stent exchange in three months.
