The video demonstrates water pressure method for endoscopic cricopharyngeal myotomy (CPOM) in a 79-year-old woman with severe, refractory cricopharyngeal dysfunction causing dysphagia and weight loss. After prior dilation and botulinum injections failed, endoscopic myotomy was performed in a very narrow hypopharyngeal space. Water jet immersion helped lift the mucosal flap, improve visualization, extend the submucosal tunnel, and control minor bleeding while reducing insufflation-related risks. The procedure was completed successfully with clip and suture closure. The patient recovered without complications and had marked symptom and swallow improvement.
