ASGE-JGES Endoscopic Submucosal Dissection ESD (Live and Virtual)

Video Transcription

Video Summary

Dr. Sergei Kantsevoy presents an overview of essential and specialty devices for Endoscopic Submucosal Dissection (ESD). He emphasizes the importance of spacer devices to create submucosal visibility and discusses various injection needles, highlighting their pros and cons. Universal knives that combine cutting and injection functions are reviewed, including the differences in injection strength and knife action, with caution on appropriate knife lengths for stomach vs. colon. Specialty knives such as IT knives, hook knives, and scissor-type knives are described, noting their specific uses and technical challenges. Bleeding control tools like Coag Graspers and hot biopsy forceps are explained, including the need for proper training and sizing. Dr. Kantsevoy advocates for dynamic, multidirectional traction devices over static ones to improve control during ESD. Lastly, he stresses the significance of defect closure post-ESD, preferring endoscopic suturing devices over clips or AX tags due to better prevention of complications like delayed bleeding and perforation.
