ASGE Masterclass: Capsule Quest – Journey Through the Future of GI Diagnostics (On-demand) | March 2025-Kickstart Your Capsule Journey: History & Anatomy Unveiled

Pdf Summary

Video Capsule Endoscopy (VCE) is a revolutionary, minimally invasive technology used for imaging the small intestine's mucosa. Historically, before VCE's introduction in 2001, diagnosing small intestinal bleeding relied on techniques such as EGD, colonoscopy, angiography, and barium studies. The development of VCE started with contributions from researchers like Iddan, Meron, Glukhovsky, and Swain around 2000, gaining traction with advancements in hardware and software.

Initially slow in advancement, VCE technology expanded significantly around 2016, incorporating AI and magnetic control, aiding in applications like acute gastrointestinal bleeding management. The capsule's technical specifications, such as size, resolution, frame rate, and battery life, vary across models from companies like Medtronic and AnX Robotics.

Training for VCE involves comprehensive education covering technology, indications, contraindications, pathology, and competency assessment, sometimes necessitating hands-on experience, upper endoscopy, and colonoscopy. European guidelines recommend a training center handle at least 75-100 VCE cases annually, with faculty experienced in interpreting hundreds of VCE reads.

Performing VCE requires specific preparations, albeit controversial. While pre-procedure purgation doesn’t significantly affect procedural outcomes, adjuncts like Simethicone may be used. Calculating capsule transit time, recording time at pylorus to cecum, and managing bleeding sites are crucial for optimal results.

In recent years, colon capsule endoscopy has emerged, serving as an alternative where traditional colonoscopy is infeasible or inadequate, demonstrating comparable diagnostic sensitivity and specificity for detecting polyps.

Overall, VCE has witnessed rapid evolution post-2016, with improved design, intelligent features, and extended applications, marking significant progress in gastrointestinal diagnostics.
