ERCP Bootcamp: Cutting Edge Comprehensive Training for the Team | July 2026-Duodenoscopes + Reusable Scopes

Video Transcription

Video Summary

Dr. Avni Jain discussed the infection risks of duodenoscopes, especially during ERCP, where complex elevator mechanisms make cleaning difficult and have been linked to MDRO outbreaks such as VRE, CRE, and resistant Pseudomonas. She reviewed current reprocessing standards: bedside pre-cleaning, leak testing, manual cleaning, inspection, high-level disinfection, drying, storage, and documentation, plus optional added sterilization steps. She explained boroscopy and newer tools like AI-assisted inspection, drying cabinets, RFID tracking, and improved scope design. Jain compared reusable scopes, disposable caps, and fully disposable duodenoscopes, noting disposable systems reduce cross-contamination risk but are more expensive and produce more waste. Clinical performance is generally similar, though some operators report differences in stiffness, imaging, and maneuverability. She recommended disposable scopes for high-risk patients or when reprocessing is uncertain, while emphasizing institutional quality control and strong infection-prevention policies.
