ERCP Bootcamp: Cutting Edge Comprehensive Training for the Team

Video Transcription

Video Summary

The session focused on wire management in advanced endoscopy, especially ERCP and EUS. Brian Noonan and Dr. Chahal emphasized that guide wires are essential for access, device exchange, and reducing trauma. They reviewed wire types, including hydrophilic, hybrid, and stiffer core wires, and explained how diameter, tip shape, length, coating, and stiffness should match the procedure and anatomy. Key themes included maintaining fluoroscopic and endoscopic visualization, avoiding blind advancement, keeping hydrophilic wires wet, preventing loss of access, and recognizing when to switch wires if cannulation or device passage fails. They also discussed long-wire versus short-wire techniques, wire compatibility with catheters and scopes, and troubleshooting difficult biliary, pancreatic, and EUS-guided procedures. The speakers stressed communication, anticipation, and teamwork, noting that successful advanced endoscopy depends heavily on coordinated work between physicians, nurses, and techs.
