In this video, a 63-year-old male with hepatic cysts and metastatic pancreatic cancer is presented. The patient had a metal stent, which became partially occluded and caused severe symptoms after chemotherapy. CT scans revealed an enlarged liver cyst and cholecystitis. Balloon dilation and placement of a pigtail stent were performed to treat the stenosis and cholecystitis. Additionally, the hepatic cyst was found to be infected, and a drainage procedure was performed using a metal stent. The patient experienced relief from pain after the procedure, and the plan is to leave all three stents in place for long-term drainage. Klebsiella pneumoniae was found in the cyst fluid and gallbladder aspirate. This case highlights the importance of considering secondary infections in hepatic cysts and the benefits of endoscopic drainage.
