In the video, a case is presented of a 76-year-old male with a history of solid food dysphagia and regurgitation. A narrow stricture was found in the esophagus, along with a large diverticulum. The differentials considered were Killian-Jameson diverticulum or Zanker's diverticulum. The plan was to perform an upper endoscopy with a possible Z-pulling procedure. Dr. Kashab used an Olympus gastroscope to visualize the septum and identified an ulcer at the bottom of the diverticulum. He performed an incision and tunneling to expose the entire septum, and then performed a myotomy using a scissor-type knife. The myotomy was completed successfully, and the Zankers diverticulum was closed using through-the-scope clips. The closure was performed in a way to induce ischemia to prevent infection and leakage. The procedure was deemed successful, and the patient's dysphagia was expected to improve.
