Hi, I'm Gary Falk. I'm a member of the faculty of the University of Pennsylvania Perelman School of Medicine, and I also had the privilege of being ASG president from 2006 to 2007. So module 10 goes over medical treatment options for eosinophilic esophagitis, and in this module we cover the following key concepts, starting with, first, what are the goals of therapy, secondly, what are your medical treatment options, and then we dive into the key treatment options, which are proton pump inhibitors, topical steroids, or biologics. We then get into how do you assess response to therapy, the data on chronic therapy, new agents under development, and importantly, what do you do if your therapy does not work. This module also gives you the information to review medical treatment options with patients going over how effective they are. We start with proton pump inhibitors, then get into the data on topical steroids, and then get into the data on dupilamab, using the best available evidence for each of these modalities. The best way to use this module is to start with these various treatment approaches that we have for eosinophilic esophagitis, and be able to be comfortable with the efficacy of each of these approaches. The case study allows you to have an example of how you would actually put this into clinical practice. So at the end of this module, what are the key things that we hope that you gain from it? Number one is to have a clear understanding of what your goals of therapy are. It's not simply to administer a drug and look at what the biopsies show. Secondly, it is to have an understanding of what you and the patient can expect to be the response to a given therapy. Thirdly, we'd like to make sure that everyone takes the time to understand how you assess response. It's not as simple as just asking a patient how do they feel. And then also, we'd like you to understand the importance that once you start a therapy In most individuals, it's probably important to continue a therapy at the lowest dose necessary to control both symptoms and endoscopic as well as histologic abnormalities. Finally, the last thing we'd like to do is give you a glimpse into the future as we are still at the beginning of therapies for eosinophilic esophagitis. There are many new compounds under development that we'd like to give you a preview for as you look into the coming years. Both Dr. Dellin and I hope that you enjoy using this module. Please feel free to reach out to either of us if there are further questions that you have after looking at this module to help you in implementation in this teaching project. Thank you.
Video Summary
In Module 10, Dr. Gary Falk discusses eosinophilic esophagitis treatment options, emphasizing therapy goals, treatment evaluation, and patient communication. Key treatments include proton pump inhibitors, topical steroids, and biologics like dupilumab. The module covers assessing therapy response, chronic therapy data, and future treatment developments. It underscores understanding therapy goals beyond drug administration and highlights maintaining therapy at the lowest effective dose while managing symptoms and abnormalities. It aims to enhance confidence in treatment efficacy, using a case study for practical application. Dr. Falk encourages questions for further understanding and implementation.
