The study explores factors driving treatment choices, barriers, and the role of shared decision making (SDM) in managing eosinophilic esophagitis (EoE) by surveying patients and caretakers. EoE, a chronic inflammatory disease affecting the esophagus, can be managed with diets, medications, like proton pump inhibitors (PPI) and topical corticosteroids (TCS), and through procedures like endoscopic dilation. Despite the existence of these treatments, there is no consensus on the optimal strategy.<br /><br />The survey involved 243 adults and 270 caregivers of children with EoE. Results indicated that preventing disease progression is a major motivator for treatment. Barriers included the inconvenience and cost of treatments, as well as adverse effects of steroids and dietary restrictions. Nearly half of respondents reported experiencing low SDM, although those treated by gastroenterologists had better SDM experiences, linked to higher satisfaction with treatment regardless of the type.<br /><br />The study highlights the need for SDM to be more widely practiced, as it correlates with increased patient satisfaction. Patients tend to be more satisfied when they are involved in the decision-making process. It also emphasizes the importance of understanding patient motivations and barriers to therapy for improving adherence and outcomes. The findings suggest that improvements in SDM could enhance patient-centered care in EoE by better aligning treatment choices with patient preferences and values, potentially leading to better management of the disease. <br /><br />Future research is recommended to validate if SDM not only leads to improved patient satisfaction but also positively impacts disease outcomes and adherence to therapy. Such studies would benefit from diverse patient samples to confirm generalizability beyond the surveyed group.
