Pdf Summary
Upper gastrointestinal endoscopy (EGD) is a vital procedure used to diagnose and treat disorders of the upper GI tract. Its usage has increased in recent years, with 7.5 million EGDs performed in the U.S. in 2019. Despite its benefits, EGD also carries risks, including bleeding, infection, and potential misuse, with 5% to 49% being performed without appropriate indication.

To ensure quality care, EGD procedures should meet quality indicators categorized into structural, process, and outcome measures. Structural measures assess the healthcare environment, process measures evaluate care delivery, and outcome measures assess results of care, such as the proportion of patients receiving appropriate treatments. By setting performance targets for these quality indicators, the goal is to maximize the quality of care delivered during EGDs.

This document is the third revision of the American Society for Gastrointestinal Endoscopy and American College of Gastroenterology quality indicators, enhancing existing metrics and introducing new ones. It aims to provide a framework for improving EGD quality across practices by integrating updated evidence and fostering consensus.

Key quality indicators include ensuring procedures are warranted by a standard list of indications, timely EGD in cases of upper GI bleeding, thorough photodocumentation, and appropriate biopsy protocols for diagnosing conditions like eosinophilic esophagitis and Barrett’s esophagus. Postprocedural indicators stress on reevaluation timelines and eradication plans for Helicobacter pylori infection.

In summary, adherence to these quality indicators is paramount for delivering high-standard care in EGD. Continuous improvement and adoption of these measures can significantly enhance patient outcomes and procedural efficiency in upper GI endoscopy practices.
