The American Society for Gastrointestinal Endoscopy (ASGE) has provided guidelines on the appropriate use of GI endoscopy in common clinical situations. These guidelines are based on a critical review of available data and expert consensus. GI endoscopy is generally indicated when a change in management is expected based on the results, when other forms of therapy have been unsuccessful, as an initial evaluation method, or when a primary therapeutic procedure is planned. It is generally not indicated when the results will not contribute to management choice or for routine follow-up of healed benign disease. Contraindications include when the risks outweigh the benefits or when patient cooperation or consent cannot be obtained. <br /><br />The guidelines also provide specific indications for different types of GI endoscopy procedures. Esophagogastroduodenoscopy (EGD) is generally indicated for evaluating upper abdominal symptoms, dysphagia, esophageal reflux symptoms, persistent vomiting, and other specific conditions. Colonoscopy is generally indicated for evaluating abnormalities on imaging studies, unexplained GI bleeding, iron deficiency anemia, screening and surveillance for colonic neoplasia, and other specific indications. Flexible sigmoidoscopy is generally indicated for screening, evaluation, and treatment of suspected distal colonic disease. Endoscopic retrograde cholangiopancreatography (ERCP) is generally indicated for evaluating jaundiced patients, suspected pancreatic or biliary duct disease, and other specific conditions. Endoscopic ultrasound (EUS) is generally indicated for staging tumors, evaluating abnormalities of the GI tract and adjacent structures, and tissue sampling. Enteroscopy, video capsule endoscopy, and natural orifice transluminal endoscopic surgery (NOTES) are also discussed in the guidelines with indications and contraindications for each procedure.<br /><br />It is important to note that these guidelines are meant to provide information to assist endoscopists in providing care to patients, but should not be considered as a legal standard of care. Clinical decisions should take into account the patient's condition and available treatment options. The guidelines may be revised as new data and technology emerge.
