Improving Quality and Safety in the Endoscopy Unit (Virtual)

Pdf Summary

The article, published by the American Society for Gastrointestinal Endoscopy (ASGE), examines radiation and fluoroscopy safety in gastrointestinal (GI) endoscopy. It highlights the necessity for proper and safe use of medical radiation due to its potential risks to both patients and healthcare professionals. Despite the crucial role of x-rays in interventional endoscopy, radiation safety protocols are often neglected, leading to significant variations in radiation doses.

Key focuses of the article include understanding fluoroscopy equipment, educating staff on radiation protection, and adhering to the "as low as reasonably achievable" (ALARA) principle. Various factors influencing fluoroscopy radiation dose, including magnification, collimation, equipment setup, and operator experience, are discussed.

The text emphasizes the importance of standardized radiation safety training for staff. It notes that many states in the U.S. lack uniform training or mandate different requirements, potentially impacting the safety of endoscopy procedures. Incorporating the ALARA principle can significantly reduce the risk of both stochastic effects, like cancer, and deterministic effects, such as cataracts, caused by radiation.

The document also outlines equipment and procedural guidelines to enhance radiation safety, highlighting the importance of wearing appropriate protective gear like lead aprons and thyroid shields. It addresses special considerations for vulnerable populations such as pregnant women, emphasizing that minimally invasive techniques and cautious approach during procedures can effectively manage these risks without banning their participation in conducting endoscopic procedures.

Finally, the advancement of AI in fluoroscopy systems is noted, contributing to decreased radiation exposure. The article calls for a comprehensive radiation safety curriculum within the endoscopy community, as done by the radiology and cardiology fields, to ensure ongoing education and quality control in radiation practices.
