Masterclass: Advanced GI Endoscopic Imaging (Live/Virtual)

Video Transcription

Video Summary

In this discussion, experts address practical aspects of upper GI endoscopy and capsule endoscopy, focusing on the use of Simethicone to clear stomach bubbles, typically administered via the instrument port rather than the flush pump to avoid biofilm risks. Pre-procedure patient ingestion of Simethicone is common and effective. Mucolytics are not routinely pre-administered but used when dense mucus is present. AI’s role in capsule endoscopy is as a diagnostic aid, highlighting suspicious frames for physician review, not replacing expert interpretation. Gastric cancer AI detection tools developed primarily in Asia face challenges in adoption in low-prevalence regions like the U.S., partly due to regulatory hurdles. Regarding gastric lesion management, depth of invasion prediction remains difficult; large excavated or nodular lesions often suggest deeper cancer needing surgery, while others may be suitable for advanced endoscopic resection. Emphasis is placed on thorough examination and good photo documentation to guide treatment decisions and minimize repeat procedures.
