Masterclass: Advanced GI Endoscopic Imaging (On-Demand) | December 2025-Optimizing Detection of Gastric Neoplasia

Video Transcription

Video Summary

The lecture addresses the challenges in detecting early gastric cancer during endoscopic exams, noting a roughly 10% miss rate and nearly 25% for synchronous lesions. Causes for missed lesions include incomplete exams, poor visibility, cognitive biases, and knowledge gaps about cancer appearance. The speaker emphasizes understanding normal gastric mucosa's appearance and recognizing specific signs of early neoplasia, such as a clear demarcation line and irregular microsurface or microvascular patterns. A systematic examination protocol covering various stomach regions enhances detection rates. Spending at least seven minutes on the exam and using pre-medications like simethicone improve mucosal visualization. Image-enhanced endoscopy techniques (LCI, BLI, NBI) outperform white light in identifying early gastric cancers. Case studies demonstrate how systematic, careful exams with enhanced imaging prevent missed diagnoses. The key to improved detection lies in thorough cleaning, structured examination, knowledge of normal versus cancerous patterns, and using advanced imaging modalities to save lives through early intervention.
