This presentation highlighted practical “pearls” that can change inpatient liver care: - <strong>Acute liver failure vs. acute-on-chronic liver failure:</strong> New encephalopathy with INR ≥1.5 and no known liver disease should be treated as <strong>acute liver failure until proven otherwise</strong>, with early transplant-center contact. - <strong>N-acetylcysteine (NAC):</strong> Beyond acetaminophen toxicity, NAC improves transplant-free survival in <strong>early non-APAP acute liver failure</strong> and can reduce short-term mortality when added to <strong>prednisolone in severe alcohol-associated hepatitis</strong>. - <strong>Transplant referral:</strong> Patients with cirrhosis should be referred at <strong>first decompensation</strong> (especially ascites) or around <strong>MELD 12</strong>, not only when MELD is high. - <strong>SBP prophylaxis:</strong> Primary prophylaxis should be limited to <strong>high-risk</strong> patients with low-protein ascites plus advanced liver or renal dysfunction. - <strong>Albumin:</strong> Use albumin for <strong>SBP</strong>, <strong>large-volume paracentesis</strong>, and selected <strong>HRS-AKI</strong> cases—not for routine non-SBP infections or to correct serum albumin. - <strong>CSPH:</strong> Clinically significant portal hypertension can be diagnosed by imaging/elastography or clinical surrogates; <strong>carvedilol</strong> can prevent first decompensation even if EGD shows no varices. - <strong>HRS-AKI:</strong> <strong>Norepinephrine + albumin</strong> is preferred over midodrine/octreotide strategies. - <strong>Variceal bleeding/coagulopathy:</strong> INR is misleading; check <strong>fibrinogen</strong> early and replace low levels with cryoprecipitate. Avoid reflexive FFP. - <strong>Platelet management:</strong> TPO agonists are for <strong>planned elective procedures</strong>, not rescue therapy. - <strong>Paracentesis in AKI:</strong> The <strong>5-L threshold</strong> is a trigger for albumin, not a hard stopping point. - <strong>PVT and refractory HE:</strong> Treat recent significant PVT with anticoagulation when appropriate; refractory encephalopathy may reflect a <strong>spontaneous shunt</strong> or <strong>TIPS-related</strong> issue. - <strong>MASLD and alcohol:</strong> Use fibrosis risk stratification for suspected MASLD, and remember there is <strong>no safe level of alcohol</strong> for health or cancer risk.
