This ASG video tip is brought to you by an educational grant from Braintree, a part of Cibela Pharmaceuticals, makers of SUTAB. This is a 46 year old woman undergoing surveillance colonoscopy. She has a history of adenomas in her distal colon. You see this lesion looks like a small diverticulum at about 20 centimeters and she has a history of extensive surgery, a total abdominal hysterectomy, as well as an operation for severe vesico-ureteral reflux as a small girl. And here's a second lesion, just proximal to the recto-sigmoid junction, we're actually looking at it in retroflexion, looks like a polyp with a little slit next to it. These are ureterosigmoid ostomy orifices, this is an old operation where the ureters are buried in the wall of the sigmoid colon to treat severe vesico-ureteral reflux, usually in kids. There's an increased risk of cancer around the sites and so surveillance is indicated. The recommendation has been to biopsy the sites and so I typically do that when I see these patients, but you have to know the surgical history. The key thing is you can't snare resect one of these sites.
Video Summary
This video tip sponsored by Braintree, a part of Cibela Pharmaceuticals, presents a case of a 46-year-old woman undergoing surveillance colonoscopy. She has a history of adenomas in her distal colon. The video highlights two lesions - a small diverticulum and a polyp with a slit - located at different areas in the colon. The woman has a history of surgeries, including a total abdominal hysterectomy and an operation for severe vesico-ureteral reflux. The lesions are relevant due to the increased risk of cancer around the surgical sites, necessitating surveillance and biopsy. It is emphasized that snare resection is not appropriate for these sites.
