  • Endoscopy Reports:  Is there a clear indication?  Are methods documented?  Any contradictions in report?
  • Screening vs. Diagnostic Colonoscopy: Documentation tips for the provider when symptoms are present in a patient that is due for screening colonoscopy.
  • Medical Necessity for Diagnostic Studies and Endoscopy:  Does your documentation support medical necessity for endoscopic treatment, capsule endoscopy and radiologic studies?
  • Infusions:  Is there a specific indication?  Were hydration services necessary?
  • Diagnostic studies:  What are payers looking for?
  • E&M services:  What is needed to support a level 5 new patient service or a level 3 initial inpatient visit?
  • ICD-10:  Does your first diagnosis listed on the claim support the medical necessity? This is the most important issue since the payer’s computer either pays or pends the claim based upon that code.
