La enfermedad por reflujo tiene una alta prevalencia a nivel mundial que varia desde un 12 hasta un 30% de los casos de acuerdo a las distintas poblaciones estudiadas. Las complicaciones de la enfermedad incluyen el esófago de Barrett, las estenosis esofágicas y manifestaciones extraesofágicas distintas, presentando todas ellas una carga economica muy importante para su atención medica. Es por ello que un diagnóstico y tratamiento oportunos son esenciales para esta enfermedad.

El tratamiento medico es considerado como el estándar de oro; sin embargo, existe una falta de respuesta o respuesta parcial a los IBP hasta en un 30% de los casos. El tratamiento quirúrgico convencional representa una excelente alternativa terapéutica, pero tiene varios eventos adversos y una no despreciable mortalidad que va desde 0.1-0.3%, es por ello que se requieren de otras alternativas de tratamiento para este tipo de pacientes. Dentro de estas alternativas, multiples tratamientos endoscópicos se han desarrollado como verdaderas alternativas tanto para el tratamiento médico como para el quirúrgico, con resultados alentadores hasta el momento. Es por ello que en esta platica revisaremos estas alternativas en sus distintas modalidades para ver cual es el verdadero papel de la endoscopía en el manejo de la enfermedad por reflujo gastroesofágico.

Reflux disease has a high prevalence worldwide that varies from 12 to 30% of cases according to the different populations studied. Complications of the disease include Barrett's esophagus, esophageal strictures and different extra-esophageal manifestations, all of which present a very important financial burden for your medical care. That is why timely diagnosis and treatment are essential for this disease. Medical treatment is considered the gold standard; however, there is a lack of response or partial response to PPIs in up to 30% of cases. Conventional surgical treatment represents an excellent therapeutic alternative, but it has several adverse events and a non-negligible mortality that ranges from 0.1-0.3%, which is why other treatment alternatives are required for this type of patients. Within these alternatives, multiple endoscopic treatments have been developed as true alternatives for both medical and surgical treatment, with encouraging results so far. That is why in this talk we will review these alternatives in their different modalities to see what the true role of endoscopy is in the management of gastroesophageal reflux disease.
