DDW 2018 Presidential Plenary Debate: Dr. Peter Draganov vs. Dr. Michael Bourke | November 2018
Released: August 2018
Viewing Time: 12 minutes
Piecemeal EMR versus ESD
What should our patient outcome goals be for endoscopic resection of large colorectal lesions?  What limitations should we consider with EMR?  What limitations should we consider with ESD? Which procedure is best for safe, efficient and complete resection of the target tissue?

Listen to Dr. Draganov review factors associated with submucosal invasion, including non-granular LST, recto-sigmoid location, increasing size and Paris classification.  Indications for use of ESD will also be highlighted:  suspected cancer with superficial submucosal invasion (non-granular LST, large granular LST) and anticipated submucosal fibrosis (prior EMR attempt, tattoo underneath the lesion and recurrent lesion).

Dr. Bourke will review why ESD is the standard of care for early gastric cancer and squamous cell cancer while also providing a rationale for why EMR is the standard of care for colon procedures.  A review of various studies and publications on wide-field EMR and ESD for colorectal lesions will be highlighted.  Dr. Bourke will suggest the rationale and need for using selective treatment algorithms for patients.

Both presentations will provide you with highlights on the use of ESD and EMR  and the factors to consider in making the right choice for the patient.

Because all browsers continually roll out new versions and updates, it is not always practical to point to a particular browser as "better" than another. Because browsers are free, you can easily shift from one to another for various purposes. Whatever browser you are using, here are some tips for better streaming and viewing of ASGE videos:

  1. Close other applications and browser tabs. This decreases the load on memory so your computer or other device can focus on streaming.
  2. Use a 5 GHz frequency if your router supports it. The common 2.4 GHz frequency is used by household appliances, so is more crowded. Avoiding it is like getting into the carpool lane on a freeway: smoother sailing.
  3. Use Ethernet instead of Wi-Fi if possible but GI LEAP is built with Wi-Fi in mind.
  4. Monitor your internet connection. If your ISP has promised you 5MBps and you are getting only 1MBps, it's time for a phone call to your internet provider to sort that out.
