Pdf Summary
The ASGE Annual Advanced Practice Provider (APP) Course 2022 will cover the topic of therapeutic endoscopy and what APPs need to know about it. The course will be presented by Dr. Vivek Kaul, an expert in the field.

Therapeutic endoscopy is a specialized field within endoscopy that focuses on the treatment and management of conditions using endoscopic procedures. It is distinct from diagnostic endoscopy, which is focused on identifying and evaluating conditions. Terms like "advanced endoscopy," "interventional endoscopy," and "therapeutic endoscopy" are often used interchangeably, but they do have some differences in their definition and application.

To become an advanced interventional endoscopist, extensive training is required. This includes 4 years of medical school, 3 years of internal medicine residency, 3 years of GI/Hepatology fellowship, and an additional 1 to 2 years of Advanced Endoscopy (AE) fellowship. There are alternative training pathways available, and the content of AE fellowships may vary between programs.

Advanced endoscopic procedures include deep enteroscopy (using single or double balloon scopes), endoscopic ablation procedures (such as radiofrequency ablation or cryotherapy), ERCP (endoscopic retrograde cholangiopancreatography), EUS (endoscopic ultrasound), EMR (endoscopic mucosal resection), ESD (endoscopic submucosal dissection), and many others.

Performing advanced endoscopy procedures requires a team of experts, including well-trained physicians, nursing staff, and techs. Other specialties, such as anesthesia, surgery, oncology, radiology, and interventional radiology, may also be involved. The APP team members play an important role in the advanced endoscopy team, including referral triage, pre and post-procedure management, complication management support, and patient and family interactions.

Advanced endoscopy procedures require specific resources and devices, such as specialty endoscopes, large endoscopy suites, fluoroscopy, and specialty electrosurgical generators. The most commonly performed advanced endoscopy procedures include ERCP, EUS, endoscopic resection (EMR, ESD, EFTR), endoscopic ablation, and enteral stenting.

The APP's role on the advanced endoscopy team includes complex endoscopy referral triage, cognitive clinical assessment, peri-procedure MD support, follow-up management, interdisciplinary care coordination, and complication management. They need to have a broad knowledge of clinical indications and contraindications for procedures, guideline-based practice paradigms, and the management of anticoagulants/antiplatelet agents peri-operatively. They also play a critical role in informed consent, research and scholarly activities, and patient education and communication.

Overall, the APP's role in advanced endoscopy is essential for safe and successful delivery of advanced endoscopy services. They are a key member of the team and their collaboration with physicians is crucial. Ongoing mentorship and professional development for APPs in advanced endoscopy is important for their continuous growth and success in the field.
